While clinical and radiological (MRI) remission is a readily achievable goal in Ankylosing Spondylitis (especially with the use of the anti-TNF biologics), and maintenance of remission possible even with tapering of medication, drug-free remission remains elusive.

Unlike in Rheumatoid Arthritis, where drug-free remission approaches 20% in those treated early and aggressively with the anti-TNFs, AS flares are the norm within a year off treatment, regardless of whether anti-TNFs are given right from the start.
