I have 2 criticisms regarding the conclusion that folic acid supplementation dose in MTX use doesn’t make a difference:
1) the Japanese famously limit MTX dose to 15mg/week, a hardly efficacious dose in this era of aggressive therapy. We routinely use 20-25mg weekly, subcutaneous even. Lower dose goes hand-in-hand with lower toxicity;
2) another arm of “no folate use” would shed light as to whether its supplementation is necessary even. I suspect not, but that could be because of low MTX dose in the first place (refer to 1).
